This is a full-time field position providing home care services across all boroughs of New York as well as Long Island and Westchester.

Cases will be clustered together.

Pay Range: $110,000.00-$125,000.00 annually

Job Details Overview: As a Registered Nurse (Float) with us, you'll play a vital role in planning, organizing, and delivering home care services across multiple locations, including all five boroughs of NYC, Westchester, and Long Island.

Responsibilities:

• Conduct comprehensive assessments of patients and families at regular intervals to determine home care needs.

• Provide thorough physical assessments and medical histories.

• Continuously reassess patient needs and adjust care plans accordingly.

• Collaborate with physicians to develop personalized care plans based on nursing diagnoses.

• Involve patients and their families in the planning process.

• Implement preventive and rehabilitative nursing procedures.

• Administer medications and treatments as prescribed.

• Offer counseling and health education to patients and families.

• Identify discharge planning needs and implement plans prior to discharge.

• Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.

Qualifications:

• Current licensure in the State of New York.

• CPR certification and a valid driver's license.

• Graduation from an accredited school of nursing.

• Preferably, a minimum of one year of recent acute or long-term care experience, ideally in an institutional setting.

• Bachelor's degree preferred, with at least one year of experience in home care.

• While management experience is not required, you will be responsible for supervising Home Health Aides and Personal Care Aides.

• Strong observation, verbal, and written communication skills.

• Excellent problem-solving abilities and basic math skills.

• Nursing skills per competency checklist.
