MDS Nurse

Deliver exceptional care with precision and compassion as an MDS Nurse at a Long-Term Care Facility in Edinboro, Pennsylvania. As a highly competent and detail-oriented professional, you will play a central role in ensuring the accuracy, integrity, and timeliness of resident assessments.

As an MDS Nurse, you will collaborate closely with nursing, therapy, and administrative colleagues to uphold the highest standards of documentation, regulatory compliance, and care coordination. Your responsibilities will include:

• Conducting and supporting comprehensive resident assessments
• Maintaining meticulous, compliant clinical documentation
• Coordinating care updates with interdisciplinary teams
• Monitoring changes in resident condition and ensuring appropriate follow-up
• Contributing to ongoing quality improvement and resident-centered initiatives

Key Requirements

• Active PA RN or LPN license
