Responsibilities:

Provide personalized home health care services to patients, including assistance with ADLs, medication management, and other essential care activities
Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care transitions and coordination
Monitor patient vital signs, report any changes or concerns to the healthcare team, and implement care plans as directed
Maintain accurate and detailed records of patient care, including noting progress, outcomes, and any changes or challenges
Collaborate with other healthcare professionals, including nurses, therapists, and social workers, to ensure comprehensive care
Demonstrate a commitment to excellence in patient care, following all necessary protocols, policies, and procedures
Stay up-to-date with industry developments and best practices to continuously improve patient outcomes and care delivery

Requirements:

1 year of experience in a healthcare setting
High School diploma or equivalent
Certification as a Home Health Aide (CHHA)
