Care Manager - Regional Float, System Staffing Office

 

Schedule varies, Days including rotation to weekend

Regularly scheduled min (1x12 hour shift p/week)

 

One would float to all six D-H system hospitals.  In this role you will have exposure to and work in the D-H Lebanon Main Campus, and the member hospitals such as Alice Peck Day in Lebanon NH, Valley Regional in Claremont NH, New London Hospital in New London NH, Mount Ascutney in Vermont, and Cheshire Medical Center in Keene NH. If you have your NH nursing license, Vermont is now a compact state so your license is valid there.

Responsibilities

  • Manages and coordinates interdisciplinary care of defined populations through the care continuum from wellness through end of life from a clinical, psychological, social and environmental perspective.
  • Identifies high risk patients requiring on-going coordination of care; performs a comprehensive patient/family assessment, develops a comprehensive treatment plan to include clinical components that will span the continuum of psychosocial issues, implements a plan of care to provide continuing support and coordination for patient/family with multiple complex system needs, manages benefits and negotiates continuing care services for enrollees in various health insurance plans, etc.
  • Utilizes innovative strategies to advocate for patient needs and negotiates complex systems to remove barriers and limitations in accessing health care in all areas including clinical.
  • Monitors the patient’s transition across and within care settings (e.g., home, clinic, skilled nursing facility, rehabilitation, hospital, etc.).
  • Shares assessment and clinical, psychological, social and environmental care plan data with patient/family consent as the patient moves through different care settings.
  • Identifies gaps in the care continuum and work with the community and provider networks to expand access to needed clinical, psychological, social and environmental services.
  • Participates in the development of clinical disease management strategies and identifies the appropriate measures for the evaluation of outcomes.
  • Participates in the development, maintenance, and coordination of an interdisciplinary care delivery system specific to individual patient needs and promotes effective resource utilization.
  • Collects and evaluates clinical and financial data/outcomes, including, but not limited to, patient satisfaction, health and functional status, and resource utilization.

    • Qualifications

    • Graduate from an accredited Nursing Program required.
    • Masters of Science Degree in Nursing (MSN) with 3 years of clinical experience required

    Required Licensure/Certifications

    • Licensed Registered Nurse with New Hampshire eligibility.
    • Current BLS required

    Pre-Admission Testing RN - Operating Room, Per Diem

    Overview The Pre-Admission Testing Registered Nurse is responsible for ensuring preoperative optimization including patient education for those patients undergoing surgical and endoscopic procedures. The nurse works closely with key stakeholders in coordinating the patients’ perioperative needs. Responsibilities Assessment and Review of Records Monitor patient medical records for preoperative testing results and exercise professional nursing judgement to identify and address barriers to optimization for surgical, diagnostic, invasive, noninvasive and other special procedures. Performs accurate and comprehensive review of clinical factors, past medical history and relevant information. Use effective interviewing skills to perform patient health interviews with surgical and endoscopy patient by collecting and analyzing clinical and psychosocial data, identifying problems and health patterns/trends which may directly affect optimization for surgical/endoscopy procedures. Promote and maintain patient safety by assessing clinical and psychological needs in preparation for the day of surgery/procedure. Clinical Knowledge Utilize clinical knowledge to ensure optimization of patients scheduled for surgery prior to their admission date to prevent surgery cancellations and delays. Utilizes clinical knowledge and established perioperative protocols (surgical and anesthetic) to provide patient with individualized preoperative education. Demonstrates the ability to identify areas of knowledge deficit, and provides additional needed information to the patient. Possess adequate understanding of operative procedure, required patient preparations, and expected post-operative course. Care Coordination/Collaboration/Communication Bring pertinent information obtain during chart review and patient interview to the care team.     Registered Nurse (RN) - PACU, Per Diem

    Overview Utilizes the nursing process in caring for all patients, to include: assessing symptoms, planning interventions, implementing treatment plans, evaluating responses to interventions, and revising plan based on overall effectiveness. Demonstrates the skills necessary to provide age appropriate care to the population served. Adheres to the established practices standards and procedure at New London Hosptial, in the administration of all medications. Makes knowledgeable decision in consideration of the facts and the disease/surgical condition Provides education to patients and caregivers based on assessment of learning needs. Creates and monitors patient environment to ensure it is safe and free from hazards. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors. Performs duties to meet the department needs to prepare patients during their time of care in surgical services. Responsibilities Pre-Operative Duties Manage patient's admission process to NLH surgical services Collaborate with anesthesia/surgeon to provide comprehensive admission care of the patient. Assess and document the status of patients' health per current ASPAN guidelines and hospital standards. Identify and report patient abnormalities to anesthesia/surgeon Ensure accuracy of patient, procedure and surgeon prior to surgical procedure. Work in conjunction with all appropriate team members to provide comprehensive care. Execute pre-procedure orders and document interventions. Assist anesthesia providers with pre-procedure interventions, document per current ASPAN guidelines and hospital standards. Oversee the work of the departments LNA. Collaborate with team members to ensure chart completeness prior to patients' surgical procedure. Post-Procedure Duties Demonstrates competence in phase 1 and phase 2 care of all post anesthesia needs of patients. Assess and documents patient condition per current ASPAN standards and hospital guidelines. Identify physical symptoms and changes and takes appropriate action in a timely manner. Reassess and documents patients response to interventions per current ASPAN standards and hospital guidelines. Demonstrates knowledge of medications and IV fluids and their correct administration, based on age and clinical condition of the patient. Demonstrates knowledge of cardiac monitoring, identifies dysrhythmias, collaborates with appropriate team members and intervenes as appropriate. Perform interventions in cases of post procedure emergencies Assess and document patient status post radiology procedure, execute physician orders, and provide post procedure instructions in conjunction with radiology staff.
