Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

  • Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed professional.
  • Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

  • Experience as a licensed professional may be deferred with a review and approval from regional leadership.
  • Medicare home health or hospice experience is preferred.

Requirements

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification

DUTIES AND RESPONSIBILITIES: 1. Ability to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes: pediatric, geriatric and the general patient population. 2. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. 3. Properly screen patients undergoing radiologic procedures for potential adverse pharmaceutical interactions and/or unnecessary pharmaceutical risks prior to procedures. 4. Administer after care to patients undergoing minimally invasive radiologic procedures. This includes proper discharge instructions. 5. Knowledge of medications and their correct administration based on age of the patient and their clinical condition. 6. Follow the five 5 medication rights and reduce the potential for medication errors. 7. Be able to start catheter and IV access as needed for radiologic procedures. 8. Access chest ports 9. Demonstrates an ability to assist Radiologist with procedures and performs services requiring technical and manual skills. 10. Demonstrates ability to perform treatments and provide services with in scope of practice. 11. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. 12. Interacts professionally with patient/family and involves patient/family in the events that are going to be involved in the imaging or biopsy of the patient.
