Responsibilities

Our PRN Registered Nurse (RN) administers skilled care visits to clients requiring intermittent professional services and teaches the client, family, and other members of the health care team. These services are performed in accordance with the physician’s orders and the established plan of care, under the direction and supervision of the Branch Director.

Qualifications

1. Must hold a current and unencumbered Registered Nursing License in the state which you intend to work.2. Must be organized, detail-oriented and possess effective communication skills both orally and in writing. 3. RN must understand the issues related to the delivery of home health care and be able to problem-solve effectively. 4. Ability to assume a flexible work schedule. Requirements:1. Must have a current driver's license and automobile insurance.

The role has no direct reporting responsibility but will function as part of an integrated, interdisciplinary patient care team. Urgent Care RN staff in these roles will help foster positive and effective working relations with senior leadership and management, local market management and business ancillary departments for the purpose of achieving the company's vision, mission and objectives. Deliver urgent, in-home, patient care under the direction of a licensed provider throughout the assigned market including initial assessments, routine follow-up visits, and ongoing care coordination. Conduct comprehensive home safety evaluations and document findings during patient visits. Deliver patient care under the direction of a licensed provider by: Obtaining patient histories Reviewing medications and treatment plans Performing physical assessments Identifying and addressing health concerns observed or reported during home visits Perform clinical interventions as appropriate and at the direction of the overseeing provider, including medication administration (PO, SL, IM, IV), wound care, point-of-care testing, phlebotomy, Foley catheter care, IV fluid administration, and other approved nursing procedures. Maintain accurate, timely, and complete patient records, ensuring documentation is updated at each patient encounter. Provide patient and caregiver education as directed by the APP-Nurse Practitioner and interdisciplinary care team. Deliver high-quality, patient-centered care while meeting established clinical, quality, and operational performance standards. Support care transitions, end-of-life discussions, palliative care interventions, and the closure of preventive care and HEDIS quality gaps, including screenings and medication adherence initiatives. Complete and submit all required documentation, reports, and administrative paperwork within established timelines. Participate in Interdisciplinary Team (IDT) meetings, providing updates and insights from patient and family interactions to support care planning. Collaborate closely with APPs, nurses, social workers, behavioral health specialists, and other care team members to develop and implement individualized patient care plans. Attend and actively participate in required training programs, including medication administration, clinical equipment use, new protocols, and ongoing professional development opportunities. Build and maintain positive relationships with local healthcare providers, community organizations, caregivers, and referral sources to support WellBe's presence and reputation within the community. Utilize available internal and external resources to address patient needs, including behavioral health services, community-based programs, and other supportive care resources. Maintain all required active, unrestricted state licensure and certifications. CPR Certification is required. Prior Telehealth experience (preferred). Home care experience (preferred). Previous experience in hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience will be considered. Possess a valid driver's license with an acceptable driving record. Requires proof of an active auto insurance policy.
