Requirements:

LPN (current and active)

BLS certificate

Able to stand for hours at a time

Able to lift up to 50 lbs

Prefer:

Experience with LTC

Experience with PCC and eMAR

Excellent communication skills and customer service skills

Tasks for this position include, but not limited to:

Able to work day shift 0600 to 1800

Safely administer medications

Perform assessments on patients for charting and when notify MD/CNP

Work with CNAs to provide good quality care to all residents

Entering orders in PCC and carrying out MD/CNP orders

Able to perform standard nursing care as learned in nursing school

Previous experience in MDS coordination in a skilled nursing or long-term care setting preferred. Knowledge of RAI process, MDS 3.0, and Medicare guidelines. Strong attention to detail and excellent organizational skills. Ability to work collaboratively with interdisciplinary teams. Proficiency with electronic health record (EHR) systems.
