The Community RN Career Direct Care Pathway placement is comprised of four levels based on a RN’s minimum experience, education, and qualifications.

Level I - 0 – Less than 12 Months of RN Experience.

Level II - 12 Months - Less than 3 Years of RN Experience.

Level III - 3 Years - Less than 8 Years of RN Experience.

Level IV - 8 Years of RN Experience.

Hours: 36 Hours- Night Shift and weekend/holidays required

Job Profile Summary

Minimum Qualifications:

1. Massachusetts RN Licensure.

2. Current Basic Life Support (BLS) Certification.

3. Department Specific: Certification(s) relevant to the department and the type of patient care being provided may be required.

4. Experience based on level (I, II, III, or IV).

1. Bachelor of Science in Nursing (BSN).

11. Supports the development of new staff members and helps facilitate their transition into unit. 

12. Guides the professional growth of department staff through precepting.

13. Participates in professional development by identifying learning needs and seeking appropriate education.

14. Participates in departmental committees as needed.

Physical Requirements: 
1. Occasional lifting of 30-35 lbs and turning patients.

2. Prolonged standing and walking.

3. Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.

4. Exposure to infectious disease and possible carcinogenic chemicals.

Dialysis Home Therapies Nurse (RN)

The Dialysis Home Therapies Nurse coordinates the administrative and clinical aspects of the home dialysis program, including home hemodialysis and peritoneal dialysis treatment options, and communicates with patients and support staff to ensure the highest standards of care. Schedule: Flexible Schedule, Monday through Friday; on call as needed Compensation: Pay range from $45.00- $60.00/ hour depending on nursing and dialysis experience. $10k Retention Bonus Benefits: Comprehensive medical, dental and vision benefits Life and long-term disability insurance provided at no additional expense to employee Paid time off (PTO) including holidays Extended Sick Bank (ESB) in addition to PTO – paid time for doctor appointments, sickness or medical leave Retirement plans with $.50 of each contributed dollar matched for eligible employees, up to 8 percent Education reimbursement Employee assistance program Wellness program Among others Responsibilities What You Can Expect: Collaborate with medical team on patient care concerns, hospitalizations, discharges and/or modality changes, etc. Coordinate home training sessions, clinic visits, patient care conferences, home visits and all clinical and educational activities to assure efficient and productive use of nursing hours and program facilities Monitor patient supply ordering and usage while maintaining quality care Communicate regularly regarding home program activities with interdisciplinary care team Monitor ongoing patient care parameters Perform all registered nurse functions and duties Create and maintain appropriate record keeping, patient training and documentation in accordance with ESRD Network, regulatory agencies, DCI CQI program and the dialysis facility's policies and procedures Qualifications Successful Candidates Bring: Excellent communication skills and a desire to teach Demonstrated clinical excellence Strong leadership skills Desire to collaborate with care teams Ability to problem solve Education/Training: Accredited nursing degree Dialysis experience a plus Current MA RN license Valid driver's license and safe driving record required DCI’s Differentiator: Since opening the first clinic 50 years ago in Nashville, Tenn., our Dialysis Clinic, Inc. family has grown to be the nation’s largest nonprofit dialysis provider with more than 270 locations in 30 states, serving nearly 14,000 patients each day.
