Licensed Practical Nurse (LPN)

The Lakes Home Care is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to work with Pediatric and/or adult patients in their own homes.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Provide skilled nursing care/services in accordance with prescribed orders on each assignment.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collect, analyze, and interpret data and information from healthcare members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred.

Rate of Pay: $28-$29 What You’ll Do: Provide nursing care to residents in accordance with established care plans, policies, and procedures. Conduct ongoing assessments of residents’ physical and psychosocial status and monitor for changes in condition. Administer medications and treatments as prescribed and document appropriately. Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers. Support health promotion, disease prevention, and wellness initiatives within the community. Ensure accurate, timely, and compliant clinical documentation. Assist with care planning, service plan updates, and resident evaluations as required. Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high-quality care delivery. Respond to resident concerns, incidents, and emergencies using sound clinical judgment. Support training and guidance for care staff as appropriate. Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services. Qualifications: Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations Minimum of two (2) years of Licensed Nurse experience required. Experience in assisted living, long-term care, or a skilled nursing environment preferred. Strong clinical judgment, problem-solving, and decision-making skills. Effective organizational, time management, and communication skills. Ability to manage multiple priorities and delegate appropriately. Demonstrated commitment to seniors and person-centered care.
