This APP will care for patients for the Walk-in Primary Care clinic and is ideal for an APP who values relationship-based care, clinical excellence, and working within a highly collaborative, supportive environment. In this role, the APP will provide comprehensive primary care services to patients across the lifespan, delivering high-quality, evidence-based care focused on health promotion, disease prevention, and the diagnosis and management of acute and chronic conditions. Responsibilities include conducting thorough patient assessments, developing and implementing individualized treatment plans, prescribing medications, coordinating care across specialties, and educating patients to support long-term wellness and self-management.
