Responsibilities

  • 1. STANDARDS OF PRACTICE RN practice is guided by the ANA Scope and Standards of Practice (2015). The Standards of Practice describe a competent level of nursing care as demonstrated by utilizing the nursing process components of assessment, diagnosis, outcomes identification, planning, implementation and evaluation. Assessment: The registered nurse collects pertinent data and information relative to the healthcare consumer's health or situation Diagnosis: The RN analyzes the assessment data to determine the actual and potential diagnoses or the issues. Outcomes Identification: The RN identifies expected outcomes for a plan individualized to the healthcare consumer or the situation. Planning: The RN develops and modifies a plan of care that prescribes strategies and alternatives to attain expected outcomes. Implementation: The RN implements the identified plan. Coordination of Care: The RN coordinates care delivery and transitions in care. Health Teaching and Health Promotion: The RN employs strategies to promote health and a safe environment. Evaluation: The RN evaluates progress toward attainment of outcomes.
  • 2. STANDARDS OF PROFESSIONAL PERFORMANCE The ANA Standards of Professional Performance (2015) describe a competent level of behavior in the professional role, including activities related to: ethics, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice professional practice evaluation, resource utilization and environmental health. The RN is expected to engage in professional role activities, including leadership, appropriate to their education and position. Qualifications

EDUCATION

 

Graduation from an accredited school of nursing. BSN preferred. All newly hired registered nurses must have current BCLS certification or must obtain within one month of hire.

 

EXPERIENCE

 

Registered Nurse is the foundational job description for all RNs. All RNs are encouraged to advance through the SPIRE clinical advancement program.

 

LICENSURE

 

Licensed and a currently registered nurse in the State of Connecticut. Clinical specialty certification highly encouraged.

PHYSICAL DEMAND

 

Requires prolonged standing; walking; use of sight, hearing, and touch. May be exposed to infectious diseases. Will be exposed to physical, mental and emotional illness as well as end of life conditions and death. Moderate stress and risk of injury from patient care. Standing/walking for more than half of the day. Frequently bending, reaching, pushing, pulling, twisting and lifting. Must be able to assist pushing/pulling/lifting patients ranging from five (5) to three hundred (300) pounds.

Monitors data regarding patient census staff productivity and collaborates with Director of Patient services to ensure financial stability. 6. Participates in the recruitment and selection of personnel. 7. Oversees the maintenance of patient clinical records and clinical staff personnel files. 8. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of patient experience. 9. Synthesizes quantitative and qualitative feedback regarding the patient experience and staff engagement to Nursing Management, staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 10. Responsibilities Assesses all patients assigned and establishes and implements the Plan of Care Adheres to policies and procedures addressing the scope of services outlined in both Infusion nursing services, pharmacy services and state laws and regulations Adheres to INS and IgNS standards of practice, CDC guidelines, scope and standards of practice, and code of ethics as defined by American Nurses Association Collaborates with pharmacy staff, sales partners, ordering provider and patient to develop and monitor goals of patients’ therapies Completes patient care documentation in place of service including patients’ home or AIS Manages vascular access devices, to include insertion of peripheral infusion devices and ability to draw labs via peripheral venipuncture Educates patients and caregivers Evaluates progress toward goals and makes recommendations to physicians for changes in the Plan of Care based on patients’ needs Coordinates nursing care with home health and other community agencies as indicated Participate in on-call rotation as scheduled per branch protocol Complete expense reports monthly tracking mileage and other miscellaneous incidental expenses Complete annual competencies and mandatory training within assigned due dates Supervisory Responsibility: No Qualifications Graduate of an accredited school of nursing; Bachelor of Science in Nursing preferred Valid nursing license in state(s) of practice Minimum of one (1) year of experience as a Registered Nurse Previous infusion and home health care experience preferred Extensive experience in IV therapy preferred Current CPR certification Flexible schedule with the availability to work a variety of shift models Ability to work effectively within a multidisciplinary team Effective verbal and written communication skills Valid driver’s license and auto insurance Must meet company standards for driving record as well as driver age requirement Percentage of Travel: 100%-Regular driving to patient’s homes for visits is required as part of this position **To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers.
