Role Description:

The Center Nurse is a clinical care team member operating within a Value-Based Care model, which prioritizes patient health outcomes and long-term wellness. Core Responsibilities:

Support patient assessment and prioritization for both walk-in and telephonic triage to ensure patients receive the right level of care and avoid unnecessary hospital visits.
Execute protocol-driven chronic condition management.
Perform skilled nursing procedures including wound care and medication administration (including vaccines).
Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care in person, over the phone, or in group settings.
Conduct medication reviews within nurse-led visits to ensure patient safety and adherence.
Conduct onsite testing (including point-of-care labs, EKGs, and specimen collection) and use patient data to identify and close gaps in preventative screenings, ensuring compliance with state scope of practice.
Review and act upon high-priority abnormal laboratory results in a timely manner to maintain patient safety and continuity of care.
Partner with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients moving from the hospital back to the clinic.
Utilize standardized protocols for medication management, prescription renewals and prior authorizations.
Ensures clinical communications are managed, tasks are completed and patient visit encounters are closed in a timely manner.
Delegates to medical assistants and phlebotomists as needed to support patient flow and efficiency.
Performs other related duties as assigned.

Implements the plan of care: Maintains constant vigilance over patients’ care to ensure that safety precautions/needs are followed (Side rails up, call lights and bedside stand within reach, exam tables in lowest position etc.); Exercises professional skills related to the plan of care; Reassesses patients as needed and appropriately revises plan of care; Correctly administers prescribed treatments; Correctly uses equipment necessary for patient care; Provides emotional support to patient and significant other; Applies scientific principles in performing procedures; carries them out safely, timely and efficiently; Makes accurate observations of patient’s conditions during treatments/procedures. Administers medications correctly and safely: Correctly identifies medication (action; dosage; side effects and implications for the patient); Meets standard on medication administration examination; Demonstrates preparation of local solutions; Demonstrates preparation of I.V. drips; Administers and documents medication correctly; Administers IVP medication as per department policy and procedure; Educates patients and significant others related to drug and food interactions. Participates in patient and family education: Provides the patient with an explanation of his/her condition as indicated; Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching; Provides for early discharge planning and appropriate referrals for post-procedural or operative care; coordinates appropriate referrals for follow-up care, specialty services, or community resources; Evaluates the effectiveness of teaching by feedback from patient/family and documents same. Duties not mentioned here, but Considered Related Are Not Essential Functions. Job Qualification Graduate from an accredited School of Nursing. Bachelor’s Degree in Nursing, preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date. Current license to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
