About the Role

We are seeking a compassionate and skilled LPN to join our weekend shift team at Southern Hills Rehab Center. As an LPN on our team, you will have the opportunity to make a meaningful difference in the lives of our residents. You will work alongside a dedicated team of healthcare professionals to provide high-quality care and support to our residents.

Responsibilities

Provide direct patient care to our residents, including administering medications, performing wound care, and assisting with activities of daily living
Monitor and report any changes in resident condition to the charge nurse or physician
Participate in the development and implementation of individualized care plans for our residents
Work closely with other members of the healthcare team to ensure seamless transitions and delivery of care
Maintain accurate and up-to-date records of resident care and treatment

Requirements

Current LPN license in the state of Oklahoma
High school diploma or equivalent required
