The Grand Rehabilitation and Nursing at River Valley is seeking a dedicated and compassionate Evening LPN to join our team of healthcare professionals. Responsibilities:

  • Provide direct patient care to residents under the supervision of RNs including assistance with activities of daily living (ADLs), vital signs, and medication administration

  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team, to meet the unique needs of each resident

  • Participate in care conferences, including discharge planning and family meetings, to ensure comprehensive care and support

  • Maintain accurate and detailed records of patient care, including medication administration, vital signs, and treatment plans

  • Collaborate with other healthcare professionals to ensure continuity of care and effective communication

  • Perform vital signs and other clinical tasks as needed

  • Assist with maintaining a safe and clean environment, adhering to infection control standards
