Reports To: Director of Nursing (DON) / Assistant Director of Nursing (ADON)
Position Summary
The Charge Nurse is responsible for supervising and coordinating nursing care provided to residents within a nursing home and rehabilitation facility. The Charge Nurse ensures high-quality patient care, compliance with regulatory standards, and effective communication among residents, families, physicians, and interdisciplinary team members. This role provides leadership to nursing staff, promotes resident safety, and supports the facility's mission of delivering compassionate, person-centered care.
Essential Duties and Responsibilities
Clinical Care
Assess residents' physical, emotional, and psychosocial needs.
Administer medications and treatments according to physician orders and facility policies.
Monitor residents for changes in condition and promptly report findings to appropriate healthcare providers.
Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
Respond to medical emergencies and initiate appropriate interventions.
Staff Supervision
Supervise and direct Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other nursing personnel during assigned shifts.
Delegate nursing tasks appropriately and ensure completion of assigned duties.
Provide guidance, mentoring, and support to nursing staff.
Monitor staff performance and report concerns to nursing leadership.
Resident and Family Relations
Communicate resident conditions, care plans, and concerns to family members and responsible parties.
Address resident and family complaints or concerns professionally and promptly.
Promote resident rights, dignity, and quality of life.
Documentation and Compliance
Maintain accurate and timely documentation in electronic or paper medical records.
Ensure compliance with state, federal, and facility regulations, including Centers for Medicare & Medicaid Services (CMS) standards.
Participate in audits, surveys, and quality assurance activities.
Report incidents, accidents, and unusual occurrences according to facility policy.
Rehabilitation Support
Collaborate with rehabilitation therapists, physicians, and other healthcare professionals to promote resident recovery and functional improvement.
Monitor residents participating in rehabilitation programs and encourage progression toward goals.
Leadership and Team Collaboration
Coordinate nursing assignments and staffing needs during assigned shifts.
Participate in shift reports, care conferences, and interdisciplinary meetings.
Assist with orientation and training of new nursing personnel.
Promote a positive, team-oriented work environment.
Qualifications
Education
Graduate of an accredited school of nursing.
Licensure
Current NJ Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing in the state of employment.
Current CPR certification required.
Experience
Previous experience in long-term care, skilled nursing, rehabilitation, or geriatric nursing preferred.
Prior supervisory experience preferred.
Knowledge, Skills, and Abilities
Knowledge of nursing practices, long-term care regulations, and rehabilitation principles.
Strong leadership and supervisory skills.
Excellent communication and interpersonal abilities.
Ability to assess resident needs and make sound clinical decisions.
Strong organizational and time-management skills.
Ability to work effectively in a fast-paced healthcare environment.
Physical Requirements
Ability to stand, walk, bend, lift, and move residents as required.
Ability to lift up to 50 pounds with or without reasonable accommodation.
Ability to respond quickly during emergencies.
Working Conditions
Exposure to infectious diseases, bodily fluids, and healthcare-related hazards.
May require weekends, holidays, evening, night, and on-call shifts.
Works in a nursing home and rehabilitation setting with residents requiring varying levels of care.
$34.00 - $39.00 per hour (LPN) or $44.00 - $49.00 per hour (RN), depending on experience.
