RN Full-time

Hospice of Western & Central Massachusetts is hiring a full-time Admissions & Intake Coordinator to join our growing agency!

This will be based out of our Agawam office with extensive travel throughout western and central Massachusetts. Candidate must be willing to travel and have a reliable and insured vehicle. Mileage reimbursed. Salaried role with weekly pay up to $48 an hour.

The admissions and intake nurse is responsible for coordinating referrals and completing admission screenings for the agency.

Requirements:

  • Must possess an active and unencumbered RN license in Massachusetts.

  • Prior hospice experience preferred.

  • At least one year of nursing experience required.

  • Experience with EMRs preferred.

Duties:

  • Receive, review, and clinically evaluate hospice referrals from hospitals, skilled nursing facilities, assisted living communities, physicians, community providers, patients, and families.

  • Gather and review relevant clinical documentation, including diagnoses, hospital records, physician notes, laboratory and diagnostic information, medication profiles, and other information necessary to evaluate hospice eligibility.

  • Communicate with referral sources to obtain missing clinical documentation and facilitate timely evaluation and admission.

  • Collaborate with the hospice Medical Director, hospice physicians, nurse practitioners, clinical leadership, and other appropriate team members regarding hospice eligibility and complex referrals.

  • Coordinate the referral process from initial inquiry through admission, ensuring timely communication with patients, families, facilities, and referral sources.

  • Perform comprehensive nursing assessments and evaluate patients for hospice eligibility based on clinical findings, available documentation, and applicable Medicare hospice requirements.

  • Complete all required admission documentation accurately and within required timeframes.

  • Obtain and document required election, consent, authorization, and related hospice admission information.
