YOUR ROLE

As a Hospice RN Case Manager, you’ll be a trusted clinical presence for your patients and their families. You’ll coordinate care, manage symptoms, provide education and support, and collaborate with physicians, caregivers, and the hospice interdisciplinary team.

Responsibilities

  • Manage and coordinate individualized hospice patient care plans

  • Perform comprehensive nursing assessments and ongoing patient evaluations

  • Provide expert symptom management and compassionate nursing care

  • Educate patients and families about disease progression, medications, and comfort-focused care

  • Collaborate with physicians, caregivers, and interdisciplinary team members

  • Maintain accurate, timely, and thorough clinical documentation

  • Provide emotional support and guidance to patients and loved ones

  • Advocate for patient comfort, dignity, and quality of life

Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4.
