Expected starting wage range is $28.43 - $35.13. Telecare applies geographic differentials to its pay ranges.  The pay range assigned to this role will be based on the geographic location from which the role is performed.  Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have) 

Graduation from an accredited Licensed Vocational Nurse (LVN) /Licensed Psychiatric Technician (LPT) program and current licensure
Current CPR/BLS certificate
Must be at least 18 years of age
Must be CPR, First Aid, and CPI certified (hands-on course) on date of employment or prior to providing direct client care and maintain current certification throughout employment

The program is staffed by a multidisciplinary team that focuses on stabilization and recovery.

Job Description The Medical Coordinator (LVN) is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - $36.00 per hour, depending on experience . Workdays may vary. Part Time - Per Diem position. Work site may vary based on operational needs. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge.
