What You Will Do to Change Lives

As a senior member of the clinical team, the Nurse Practitioner provides direct psychiatric nurse practitioner care and medical treatment to members served.

Nurse Practitioner, PMHNP - Per Diem as needed, 10 hour shifts onsite

Compensation -

Expected Employee starting wage range - $78 - $96 / hourly

Telecare applies geographic differentials to its pay ranges.  The pay range assigned to this role will be based on the geographic location from which the role is performed.  Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• Bachelor's Degree in Nursing Science and an advanced degree in a healthcare science as required by the State Licensing program

 Minimum of one (1) year of full-time experience, or its part-time equivalent, working in a program serving persons with mental disabilities

• Completion of an accredited Nurse Practitioner program

• American Nurses Credentialing Center (ANCC) Board Certified as a Psychiatric Mental Health Nurse Practitioner (PMHNP)

• Licensure as an Advanced Practice Nurse Practitioner in the State of practice

• Active DEA and Furnishing licenses

Individuals can stay in the CSU for up to 24 hours. Children and adults are treated in separate areas. Admission is voluntary or involuntary (5150).

Comprehensive Health Assessment: Conducts thorough patient evaluations by obtaining detailed medical histories, performing age-appropriate physical examinations, and ordering or interpreting diagnostic tests to guide clinical decision-making. 4. Clinical Reasoning & Diagnosis: Applies critical thinking to synthesize and analyze clinical data, formulating accurate differential diagnoses while integrating patient history, physical findings, and diagnostic results. 5. Personalized Treatment Planning: Develops and implements individualized, evidence-based treatment plans incorporating pharmacologic and non-pharmacologic interventions, patient education, and interdisciplinary referrals as needed. 6. Ongoing Patient Evaluation: Monitors patient progress through follow-up assessments, adjusting treatment plans as necessary to ensure therapeutic effectiveness and alignment with patient-centered goals. 7. Scope of Practice & Collaboration: Provides care in alignment with the Nursing Practice Act, scope of service, and hospital policies, consulting with physicians and specialists as needed for comprehensive patient management. 8. Accurate & Timely Documentation: Ensures meticulous documentation of all patient encounters, assessments, interventions, and outcomes in accordance with hospital and clinic policies to maintain compliance and continuity of care. 9. Quality Improvement & Professional Engagement: Actively participates in quality assurance and performance improvement initiatives, contributing to hospital and clinic efforts to enhance patient safety, care efficiency, and clinical outcomes. 10.
