Licensed Practical Nurse, Part Time NOC Shift

A licensed practical nurse (LPN) is a technical nurse who has been educated to perform nursing intervention under the direction of a registered nurse in compliance with the nursing service policies and procedures. This position has responsibility to deliver selected treatments/procedures/tests; medications and patient care as delegated and directed by the RN.  The LPN assists the RN to monitor the care provided by the nursing assistants, including personal care and restorative nursing programs.  The LPN is assigned to a neighborhood with responsibility for 16 to 32 residents.

40% - Administer medications and performs treatments.
1. Administers medications prescribed for routine administration including those given via oral, subQ, IM, nasal, eye, ear, vaginal, rectal, inhaler and nebulizer routes. Observes the effects of medications, reports to the RN, RN Supervisor and MD any unusual or adverse reactions. Assures compliance with the State and Federal guidelines related to medication pass.
2. Administers IV therapy and IV prepared piggyback medications and installs heparin and saline flushes- if the LPN holds a facility approved certification and after competency has been confirmed. 
3. Administers PRN medications as indicated, consulting with the RN as needed. 
4. Performs treatments after skills training including: clean and sterile dressing changes, tracheostomy care and changes, skin tear protocol, catheter care and changes, colostomy care, blood sugar checks, G/J tube care and feeding, oxygen administration, suctioning and nebulizer use.
5. Performs EKGs and bladder scans and vital signs. 
6. Completes lab draws and maintains annual competency. 

25% - Collects and records data which will be incorporated into nursing assessments and participates in the overall implementation of the resident's plan of care.
1. Prepares incident/injury reports, weekly summaries, vital signs and physical symptoms, response to analgesics and other medications.
2. Immediately reports resident changes in condition to the RN and RN Supervisor
3. Assists the RN to provide guidance and supervision to the nursing assistants.
4. Participates in the overall collection of data, planning and implementation of the care plan, CNA plan of care sheets and restorative plan of care.
5. Documents medication administration, treatments, treatment progress notes, monthly summaries and restorative/medicare nursing notes, quarterly assessments and data gathering related to the resident's health status.

25% - Monitors and assists with resident dining.
1. Assures that the care needs of high-risk residents are followed
2. Assists feeding residents at risk for swallowing concerns.

5% - May function as the team leader on the unit.
1. May function as the team leader during code situations (Code Blue, Code Black, Code   Orange, Code Yellow, Code Grey).
2. Administers emergency care including CPR and use of the AED.
3. Assures safe environment for residents, staff, families and visitors. 
4. Participates in Quality Assurance and/or other committees as assigned
5. Attends and completes required inservices or online training. 

5% - Perform other duties as assigned.


 
  • Current theory base in medical, geriatric and psychiatric nursing.
  • Current theory in medication administration including psychoactive medications.
  • Knowledge of needs of client populations and treatment sources.
  • Ability to communicate effectively orally and in writing.
  • Ability to effectively present information and respond to questions from managers, employees, residents, families, professionals and the general public
  • Ability to work well as a team member.
  • Skill in reviewing clinical work of others according to professional standards and practice guidelines.
  •  Demonstrates responsibility and accountability for professional practice and for continued professional growth and self evaluation.
  • Ability to maintain professional appearance and conduct at all times. 
  • Effectively copes with typical job stress. 
  • Performs related work as assigned. 
  • Licensed Practical Nurse with current Wisconsin Licensure in good standing.
  • Long-term care experience preferred.
  • Completion of the expanded role LPN skills preferred.
  • CPR Certification or evidence of continued attendance at CPR updates
  • May need to stay past the normal shift until relieved in order to meet minimal staffing requirements.
  • Ability to sit, stand, bend, and stoop for periods of time.
  • Ability to lift up to 50 pounds occasionally.
  • Ability to work in a stressful environment.
  • Ability to respond to emergency/crisis situations.
  • Ability to push/pull a medication cart.
  • Exposure to noise.
  • Exposure to blood and/or body fluids.
  • Uses personal protective equipment as needed but not limited to: gloves, gowns, goggles and protective shield.
  • Ability to enter accurate information into charts and reports.
  • Work is inside in a skilled nursing home facility.
  • May be subject to potentially hostile and emotionally upset residents, family members, personnel, visitors,   etc.
  • May be subject to falls, burns, odors, etc., throughout the work day.
  • May be exposed to infectious waste, diseases, blood, body fluids, conditions, etc.
  • CPR/AED certification required
