Empath Home Health, a member of Empath Health is seeking a compassionate part-time Weekend Baylor RN Registered Nurse (Home Health) to work with our Home Health team to deliver individualized care to patients in their homes throughout North Hillsborough & East Pasco counties (North Tampa, Dade City, Land O'Lakes, and surrounding areas).

Locations: Home health visits in North Hillsborough & East Pasco counties (including North Tampa, Lutz, New Tampa, Tampa Palms, Wesley Chapel, Land O’ Lakes, Dade City, San Antonio, and surrounding areas).

Schedule: This is a part-time Weekend Baylor role requiring full weekends. Saturday and Sunday (20 hours per week).

What You’ll Do

  • Perform comprehensive patient assessments, including OASIS assessments at required intervals
  • Develop, initiate, and revise individualized plans of care as patient needs evolve
  • Deliver skilled nursing services and treatments as outlined in the plan of care
  • Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need 

  • Graduate of an approved nursing program with an active Florida RN license
  • Prefers BSN or MSN degree
  • Minimum of 1 year of direct patient care experience
  • Home Health RN experience required
  • Strong assessment, critical thinking, and patient education skills
  • Willingness to participate in weekend and holiday on-call rotations
