What You’ll Do:

  • Work with the interdisciplinary care team to ensure accurate patient assessment and care planning

  • Maintain open and ongoing communication with patients and families

  • Provide compassionate and appropriate nursing care

  • Assist with medications and treatments

  • Perform other patient care duties as assigned

What We’re Looking For:

  • Missouri Licensed Practical Nurse (LPN) license

  • Caring, compassionate, and positive attitude

  • Team player who works well in a collaborative environment

  • Prior Matrix and SNF experience is a plus
