Job Summary

Mountain View Rehabilitation and Senior Living Center, a premier residential, long-term, rehabilitation, and subacute care provider, is seeking a dedicated and compassionate Weekend LPN (Baylor Program) to join our team! As a Weekend LPN, you will play a vital role in delivering high-quality care to our residents, working closely with our interdisciplinary team to ensure a holistic approach to their well-being.

Responsibilities

  • Provide direct patient care, including administering medications, changing wound dressings, and performing vital signs

  • Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans

  • Monitor and document patient progress, reporting any changes or concerns to the healthcare team

  • Assist with activities of daily living (ADLs), such as bathing, dressing, and grooming

  • Participate in facility-wide quality improvement initiatives to improve patient outcomes and satisfaction

  • Provide emotional support and companionship to residents, promoting a sense of comfort and well-being

  • Maintain a clean and organized environment, adhering to infection control protocols
