Registered Nurse (RN - Hospital) - PCU

STATEMENT OF PURPOSE

Coordinates total nursing care for patients of all ages utilizing the nursing process; directs patient and family teaching; provides leadership by working cooperatively with ancillary nursing and other patient team personnel in maintaining standards for professional nursing practice in the clinical setting.


Full-time, benefited position working 72 hours per pay period. Days of work are Sunday-Saturday. Scheduled shifts are day/night rotation with occasional evening shifts. Scheduled weekend rotation will be 12 hour shifts every third weekend. Holiday work is every third holiday per year. Hours of work are 7:00am - 7:30pm, 7:00pm - 7:30am, and occasional 7:00am - 3:30pm, 11:00pm - 7:30am, and 3:00pm - 11:30pm. Will cross-train to ICU after demonstrating competence on PCU.    


JOB FUNCTIONS

  • Maintains the standards of nursing care and implements the policies and procedures of the hospital and Clinical Services.
  • Provides direct patient care for assigned patients of all ages including pediatrics, adult and geriatrics, utilizing the nursing process including assessment, planning, intervention and evaluation.
  • Assesses the patient's condition considering age-specific developmental needs, educational needs of patient and family and discharge planning needs.  
  • Develops a plan of care for the patient and prescribes appropriate nursing interventions to achieve expected patient outcomes.
  • Evaluates patient outcomes and modifies plan of care as patient’s needs or condition changes.  
  • Directs and participates in discharge planning and patient and family/significant other education.
  • Reports pertinent observations and reactions regarding patients to the appropriate person (i.e., physicians, Unit Supervisor, and/or Nursing Manager) and records those observations accurately and concisely.
  • Delegates nursing care to other health care members in accordance with the patient's needs and the nursing personnel's capabilities and qualifications. 
  • Administers medications, IV therapy and performs treatments for assigned patients as ordered by the physician.
  • Assists physicians with special tests and procedures such as thoracentesis, lumbar tap, etc.
  • Ensures that equipment is maintained in a clean and safe manner.
  • Participates in orientation of new personnel and performance evaluations of nursing staff.
  • Coordinates nursing care of patients when scheduled for therapy or procedures by other departments.
  • Establishes and maintains effective communications with patients, families and staff to contribute to the hospital mission.
  • Initiates and/or assists in emergency measures for sudden adverse developments in patients such as cardiac arrest.
  • Participates in the maintenance of a clean and safe environment.
  • Participates in staff development programs.
  • Performs other related duties as assigned or requested.

QUALIFICATIONS/REQUIREMENTS FOR THIS POSITION

  1. Preparation and Training - Work requires a professional level of knowledge acquired by completing an accredited RN nursing education program. Must hold RN licensure from the ND State Board of Nursing for the current year. ED staff nurse position requires PALS/ACLS certification.
  2. TNCC is highly encouraged. ICU staff nurse position requires completion of a basic cardiology class for nurses and ACLS certification. Family BirthPlace (FBP) staff nurse position requires completion of a basic fetal monitoring course and neonatal resuscitation course (NRP). Position requires completion of BLS CPR. Position is required to attend Crisis Prevention Intervention (CPI) training in the appropriate timeframe according to department orientation checklist
  3. Work Experience - Previous work experience preferred but not required for staff RN positions in Patient Care Unit (PCU). (See exceptions indicated below). Time to reach competency on the job: 3 weeks to 3 months for entry level staff RN. ICU Staff RN, FBP Staff RN, ED Staff RN and PACU RN positions: Preference for a minimum of one year nursing experience following completion of RN nursing education program. Time to reach competency on the job: 3 weeks to 3 months following orientation to the work area. 
  4. Attendance - Punctual and regular attendance is an essential responsibility of each employee at JRMC. Employee are expected to report to work as scheduled, on time and prepared to start working. Employees are also expected to remain at work for their entire work schedule.
  5. Analytical Ability - A high level of analytical ability is required in a variety of duties in order to gather and interpret complex data and to find solutions to difficult problems.
  6. Independent Judgement - Independent nursing judgment is often required by the professional RN based on established policies and procedures. Substandard performance in nursing skills or judgment may adversely affect patient care, patient welfare and hospital goodwill.
  7. Contact with Others - Very frequent contact with others. Must demonstrate social sensitivity and the ability to communicate effectively in a professional manner.
  8. Supervising the Work of Others - May be required to plan, assign work to and check the work of 1 to 5 nursing personnel doing related work on the shift. Supervision is provided to LPNs, Nurse Assistants, and Unit Clerks.
  9. Responsibility for Welfare Others - High frequency with which work performed by the staff RN may result in very serious physical or emotional harm to others. It is the responsibility of the professional RN to continuously uphold the highest degree of performance within currently defined and acceptable nursing practice standards.
  10. Mental / Visual Effort - Must be able to speak and write the English language in an understandable manner. Must demonstrate physical and emotional ability to meet agreed upon work schedule and performance standards required by the position. Required duties are such that concentrated attention must be applied on demand consistent with manual skill requirements in situations that may at times be stressful. Visual acuity necessary for reading charts and working with medical devices; color perception necessary for interpreting testing procedures involving color comparisons.
  11. Physical and Sensory Effort - Frequent standing or walking. Lifts, positions, pushes and/or transfers patients and/or equipment. Frequent reaching, stooping, bending, kneeling and crouching. Needs finger dexterity when handling medication, specialized equipment and performing nursing assessments.
  12. Working Conditions - OSHA job classification: This position is defined as a Category I in which the employee does have exposure to bloodborne pathogens. Position requires exposure to patients with infectious disease and odor. Must practice aseptic techniques and observe Standard Precautions. Participates in and complies with JRMC Safety Management program. Position is subject to frequent interruptions. Work may be on weekends and holidays; beyond normal working hours and/or temporarily in other positions when necessary.

Location: Moody Outpatient Center 5th Floor Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, that focuses on a particular patient population(s)/specific health conditions, by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. Responsibilities: Serves as a patient advocate, by focusing on patient needs, patient rights, confidentiality, and religious and cultural preferences. Maintains positive working relationships with all Parkland internal and external customers, employees, etc. Serves as a resource for patients and other healthcare team members. Provides patient care educational information and training to patient and family members. May serve as a resource for specific clinical patient care issues. Provides appropriate hand off any time care is relinquished. May participate in community outreach activities (such as health fairs, community collaborations or outreach clinics) as assigned. Maintains and implements knowledge of applicable rules, regulations, policies, laws and guidelines that impact or govern nursing practice, and ensures the hospital and Parkland are in compliance. Develops effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Maintains and updates knowledge of sanitation, infection control, safety, supplies, equipment, appropriate utilization of supplies and materials. Performs appropriate patient assessments including continuous surveillance. Recognizes changes in a patient's condition and provides appropriate interventions as necessary. Consults with other team members as required, notifies providers to report changes in patient condition and implements orders in a timely manner. Exercises independent judgment in providing safe nursing care to the critically ill/stable patient. Must be able to manage an adult ICU patient with neurological, pulmonary, cardiovascular, gastrointestinal, genitourinary disorders/conditions. Must provide safe medical administration using the patient's six (6) rights, provide maintenance and management of IV access, must be able to perform POC monitoring and demonstrate safe infection control practices. Supports pain management as appropriate for each individual patient, provides emergency resuscitation care of an adult and provides an individualized plan of care. May be required to perform CRRT for the adult ICU patient. Completes appropriate documentation. Maintains accurate and timely entries and notations in patient care records and hospital documents. Ensures that all records and documents meet the standards, goals, and objectives of the Adult ICU/Critical Care department and Parkland. Identifies ways to improve work process and improve patient and customer satisfaction. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall goals of the Adult ICU/Critical Care department and Parkland. Stays abreast of the latest developments, advancements and trends in nursing practice by attending seminars/workshops, reading professional journals and actively participating in professional organizations. Maintains State of Texas Licensure as a Registered Nurse, plus any additional required licensures/certifications. Integrates knowledge gained into current work practices. Seeks and utilizes direction from more experience staff and team members as needed. May work with other staff in providing direction to non nursing staff. Participates in information exchanges with nursing and other Parkland staff that support the standards, missions, goals and objectives of the Adult ICU/Critical Care department and Parkland.
