As a Registered Nurse or Licensed Practical Nurse, you will:

Assess residents and help develop care plans.
Administer medications and treatments.
Provide direct nursing care, including wound care and IV therapy.
Coordinate care with physicians, families, and the healthcare team.
Support and guide CNAs and other care staff.
Educate and support residents and their families.

Open Shifts Available:

2nd shift, Full-time, 8 hour shifts Rotating Weekends

Qualifications:

State of Iowa RN or LPN License

Multi-Site MDS Coordinator - RN/LPN

Key Responsibilities Complete, review, and submit MDS assessments, CAAs, and comprehensive care plans in accordance with CMS RAI guidelines. Manage MDS schedules, ARDs, Medicare assessments, IPA reviews, and Medicaid Case Mix processes. Ensure accurate clinical documentation, ICD coding, and regulatory reporting. Monitor MDS quality measures, validation reports, and compliance metrics; complete corrections as needed. Educate and support nursing and interdisciplinary team members on MDS best practices and documentation standards. Participate in Medicare meetings, Triple Check processes, QAPI initiatives, and quality improvement activities. Identify significant changes in resident condition and coordinate appropriate assessments. Travel to assigned communities to provide onsite support, training, and coverage as needed. Qualifications Graduate of an accredited nursing program. Current Iowa RN or LPN license in good standing. Minimum 2 years of long-term care experience . Minimum 2 years of dedicated MDS experience .
