Job Summary and Qualifications

The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other members of the management team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization’s vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.

What you will do in this role:

  • Assists with admission and discharge processes to ensure efficient throughput and high quality, patient-centered care.
  • Participates in the ongoing assessment of the quality of patient care services provided in the unit, in collaboration with other members of the management team.
  • Collaborates with subject matter experts and other managers to create an environment of teamwork that supports improved outcomes and service.
  • Supports a patient-first philosophy and engages in service recovery when necessary.
  • Supports the efforts of the facility to improve engagement by operationalizing current nursing strategies, including employee rounding, hourly rounds, and other initiatives.   
  • Provides recommendations related to interviewing, selecting, and training new staff. Recommends and implements courses of action, including training and development, conflict resolution, personnel policy compliance, completion of performance evaluations, and/or disciplinary actions to ensure a competitively better organization.
  • Assists with staff scheduling. Manages all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines.
  • Supports proper inventory control and assists with managing supplies and equipment.

What qualifications you will need:
  • Basic Cardiac Life Support
  • Must hold a TX RN license or a compact license
  • Bachelors Degree in Nursing required
  • 2+ years experience in acute hospital experience
  • 1+ years RN leadership experience
  • 2+ years of Labor and Delivery experience
  • Advanced Fetal Monitoring, or Intermediate Fetal Monitoring, must be obtained within 6 months of employment start date

Assesses and interviews patient and caregivers as part of this evaluation and as needed. Participates in multidisciplinary rounds (MDR’s) to help identify current length of stay (LOS), expected discharge date, anticipated discharge disposition, barriers to discharge, avoidable days, and potential denials. Communicates with the multidisciplinary team, patient, family, and post-acute care stakeholders to coordinate care. Assists in the determination of the level and type of care needed; coordinates/facilitates patient care progression throughout the continuum with the objective of enhancing quality clinical outcomes and safe discharge planning. Schedule/coordinate patient clinical needs to the appropriate post-acute care facility based on facilities’ clinical capabilities/offerings, historical quality outcomes results, preferred network, and patient informed choice. Serves as a content expert regarding payor information. Educates the multidisciplinary team, patients and caregivers regarding payor requirements and barriers. Communicates with payors as needed to coordinate care. Responsibilities Job Summary This position requires a thorough understanding of the facility’s treatment program and philosophy, admission process and criteria, support and administrative services, medical knowledge of various aspects of substance abuse and dual diagnosis patient care. The Admissions Nurse is a member of the treatment team providing professional level of nursing care to patients under minimum supervision and consultation. Identify client care requirements by establishing personal rapport with potential and actual clients and in partnership with other persons in a position to understand care requirements. Promote client's independence by establishing client care goals, teaching client to understand condition, medications and self-care requirements, and answering questions. Assure quality of care by adhering to therapeutic standards; measuring health outcomes against client care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by state board of nursing, state nurse act, and other governing agency regulations. Document patient care services by charting in client and department records. Maintain nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing orders for supplies. Knowledge of mental health, addiction, and addiction treatment preferred. Detail oriented and able to work in a fast paced environment.
