Overview
Responsible for promoting the integration of services and continuity of care of diabetes patients. This includes the consultation with providers, assessment, recommending appropriate interventions, evaluation of glycemic management, coordination of educational needs, and enhancing the growth of related diabetes programs. Works with Program Director to achieve program objectives and enhance clinical outcomes and patient satisfaction.
