JOB LOCATION : Main Campus

DEPARTMENT: CASE MANAGEMENT

SCHEDULE: Full Time, 8 HR Day Shift, 8

Assesses, plans, implements, coordinates, monitors and evaluates potential options and services to meet an individual’s health needs through communication and utilization of available resources to promote quality and cost effective outcomes.

Knowledge, skills and abilities:

RN or other applicable clinical licensure required. Bachelor’s degree and/or certification in Case Management strongly preferred. Previous experience desired.
Recent sound clinical experience of 3-5 years. Must have extensive experience in a particular patient population or moderate experience in a broad population.
Documented competency in assigned patient population including age specific competency.
Demonstrated expertise in management, problem solving, critical thinking, and data management.
Knowledge of medical literature, research methodology, health care delivery systems, financial/reimbursement issues, utilization management, and the medical center/medical staff organizations.
Computer skills desirable (must be willing to learn spreadsheet analysis, statistical functions, word processing). Ability to understand larger scope of hospital-wide information systems.
Excellent written and verbal communication skills. Ability to influence decisions, gain cooperation, and
facilitate change with physicians, all levels of management, employees and supervisors of other hospital Departments.

Working Conditions:

Requires considerable sitting, standing, mental effort, stooping, bending, lifting and pushing. Requires operation of office equipment. Works in a high stress environment.

The APRN may practice only as prescribed in their job description, and as defined in the SGMC Health credentialing process. The APRN may perform advanced nursing functions and certain medical acts which include, but are not limited to, ordering drugs, treatments and diagnostic studies such as provided in O.C.G.A 43-34-26.1 and Chapter 41-0-13. The APRN may perform those tasks which they are qualified to perform, for which the sponsoring physician has privileges, and for which protocols signed by the supervising physicians exist. The APRN participates in the QA and PI process and provides education and consultation services in area of expertise. KNOWLEDGE, SKILLS & ABILITIES Graduate of an accredited, post-basic educational program for nurse practitioners. Georgia Nurse Practitioner License Certification by the American Nurses Credentialing Center or American Academy of Nurse Practitioners. Maintains biennial renewal of Board Authorization to practice as a Nurse Practitioner. CPR Certification ACLS Certification within 180 days of employment. Preferably at least four years of health care employment. Flexible in work schedule, assignments, workloads, setting priorities and utilizing problem solving techniques. Ability to analyze regulations, standards, clinical issues, and patient outcomes. Excellent verbal, written, and behavioral communication skills. WORKING CONDITIONS - ADA INFORMATION Works primarily in hospital setting, including clinical/patient care areas.
