Job Description

Your Impact: 

  • Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home. 
  • Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients. 
  • Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions. 
  • Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital. 

Job Requirements

What You Bring: 

  • Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, Acute Care and/or Family. 
  • State Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP). 
  • Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.

Description The OB First Assist collaborates with the surgeon and the healthcare team in performing a safe operation with optimal outcomes for the patient.
