Job Overview

Responsible for the development and implementation of homecare services for program participants, including the coordination of all contracted home services, durable medical equipment, and nursing home care. Attending scheduled IDT and Coordination meetings.   

Minimum Requirements

  • Graduation from an accredited LVN school, coupled with a current LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians required.

  • A minimum of one (1) year of experience practicing as a Licensed Vocational Nurse.

  • Experience and knowledge regarding the physical, mental, and social needs of the patients are highly desirable.

  • At least one year of working with a frail or elderly population

  • Valid California driver’s license with a good driving record.

  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Pay for this job starts at $31.93 hourly

We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - 36.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3.00 graveyard shift differential . The schedule for this position is 7am to 3:30pm, 5 days on, 2 days off. Sunday - Thursday. Work site may vary based on operational needs. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge. Link clients to medical services when necessary (insurance, PCP, specialists, referrals). Coordinate home health visits (if needed) and follow-up on medical appointments. Promote health literacy by providing patient education with regards to clients’ illnesses, medications, preventative measures, treatment management and overall general health.
