We are seeking a skilled Registered Nurse Case Manager to join our team. This role is crucial in assessing patient needs, coordinating care, and ensuring effective communication among all parties involved in patient care.

Responsibilities:
  • Manage Nursing Care: Oversee the coordination of nursing care, home health aide services, and applicable therapies once approved by the Department of Labor.
  • Ensure Confidentiality: Maintain confidentiality of all client and office records in accordance with HIPAA guidelines and the Privacy Act PHI.
  • Facilitate Communication: Ensure effective communication and collaboration among the care team to provide cohesive and comprehensive care.
  • Additional Duties: Perform other duties as assigned to support the team and enhance patient care.
Qualifications:
  • Current RN License: Valid and active Registered Nurse license in good standing.
  • Experience: At least one year of experience in a home health setting, or related area, with strong assessment skills.
  • Organizational Skills: Excellent organizational and time management skills to handle multiple clients efficiently.
  • Tech Savvy: Proficient in computer and internet-based applications, as well as office equipment.
  • Equipment Knowledge: Competent working knowledge of client-based equipment.
  • Background Check: Must pass a criminal background check and sanction screening.
  • Professional Appearance: Professional demeanor and appearance are necessary.
Pay Range: USD $45.00 - USD $48.00 /Hr.

Registered Nurse Coordinator - Chemotherapy

Summary: The Lung Nodule Coordinator leads and facilitates the development, implementation, and ongoing adherence to identifying and treating lung nodule patient populations. The Lung Nodule Coordinator works under the supervision of the Director of Pulmonology, Clinic Manager, Pulmonary Specialists, and Cardiothoracic Surgeons. This position develops systems of care that monitor patient progress and promote early intervention. Work effectively with other healthcare team members to optimize interventions and manage community outreach to promote disease awareness and the offerings of the specific clinic focus. The Lung Nodule Coordination will manage the utilization and practice metrics to refine the delivery of care model to maximize clinical, quality, and fiscal outcomes. The RN Coordinator also ensures these programs adhere to the mission and vision of CSV. Responsibilities: Assume responsibility for the clinical coordination and consultation regarding care across the continuum for all lung nodule patients. Consult and partner with nursing leadership as needed to achieve desired patient outcomes. Assess the educational needs of staff and assist in developing educational programs, staff education materials, and community education as needed. Maintain and ensure compliance with continuing lung nodule education for medical and nursing staff. Ensure evidence-based screening and treatment protocols are the standard of care within the facility.
