About the Role

As a Licensed Practical Nurse (LPN) in Home Health, you will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse and in collaboration with an interdisciplinary care team. You'll play a vital role in helping patients manage their health conditions, maintain independence, and improve their quality of life.

This position is ideal for nurses seeking a flexible weekend schedule while continuing to provide exceptional patient-centered care.

 

What You'll Do

  • Provide skilled nursing care according to individualized care plans.
  • Administer medications and treatments safely and accurately.
  • Monitor patient conditions and report changes to the supervising RN.
  • Collaborate with physicians, case managers, caregivers, and other healthcare team members.
  • Educate patients and families on disease management, medications, and treatment plans.
  • Maintain accurate and timely clinical documentation.
  • Support patients with dignity, compassion, and respect.
  • Participate in care conferences, team meetings, and ongoing training as required.

 

What We're Looking For

 

Required Qualifications

  • Graduate of an accredited Practical Nursing program.
  • Current, unrestricted Massachusetts LPN license.
  • Successful completion of NCLEX-PN.
  • Current CPR certification.
  • Strong clinical assessment, communication, and documentation skills.
  • Valid driver's license, reliable transportation, and automobile insurance.
  • Ability to meet required health screening and clearance requirements.
  • Professional references demonstrating clinical competence and reliability.

 

Preferred Qualifications

  • Associate degree.
  • Two or more years of nursing experience.
  • Experience in home health, hospice, long-term care, community health, or related settings.
