Job Overview

The LVN provides direct patient care under the direction of a Registered Nurse or Physician. The LVN tasks include vital signs, phlebotomy, gathering data, and medication administration. The LVN verifies medication for Medical Assistants within the LVN's scope of practice.

Minimum Requirements

Graduation from an accredited LVN school with a current LVN license issued by the State of California, Vocational Nursing and Psychiatric Technicians.
Preferred 1 year of providing patient care outpatient/inpatient, Skilled Nursing, Home Health, or clinic setting.
IV certified preferred.
Preferred wound care experience.
Experience and knowledge regarding the physical, mental, and social needs of the patients.
Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

IHH currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The Medical Coordinator (LVN) is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - 36.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3.00 graveyard shift differential . The schedule for this position is 7am to 3:30pm, 5 days on, 2 days off. Sunday - Thursday. Work site may vary based on operational needs. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs.
