Hours: Monday through Friday 8:30am - 5:00pm

This Certified Nursing Assistant will be responsible for, but not limited to:

Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
Feeds and assists with meals.
Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education:

High School Diploma or equivalent

Professional Experience:

Must have a State Certified CNA license
Experience in hospice or home health, preferred
No experience necessary but 1 year, preferred
Must be willing to travel
Must have a valid driver's license and reliable transportation

This position will assist in patient care throughout the nursing areas and/or stay suites is responsible for the safe transport of patients via wheelchair or stretcher, dispatch services and appropriate documentation of services. Provides the services of an observer for any patient on a patient care unit who has been assessed to require constant observation to maximize his/her safety. Performs general receptionist and clerical duties to facilitate efficient functioning of the nursing unit and/or stay suites to post ASC patients. Observe skin changes and reports to RN Promote infection control by using appropriate hand washing techniques. Follow standard precautions and any other isolation precautions as dictated by patient condition. Maintain a safe environment and protects patients from injury by observing the patient who may be at risk for falls, confusion, disorientation or suicidal behaviors.
