At A Place At Home – Merrimack Valley, we're hiring compassionate caregivers for weekday morning shifts (3–6 hours in length) throughout Chelmsford and surrounding communities. Whether you're looking for "mother's hours," one or two shifts each week, or a full-time schedule, we'll work with you to find hours that fit your life.

What You'll Do

Our clients typically need a mix of the following services:

  • Personal care: Assist clients with bathing, dressing, grooming, and mobility

  • Companionship: Provide companionship and meaningful conversation

  • Housekeeping & meal prep: Prepare light meals and assist with light housekeeping

  • Medication reminders

What We're Looking For

  • Compassionate, dependable, and professional attitude

  • HHA or CNA certification preferred

  • Reliable transportation

  • Clear communication

  • Home care experience strongly preferred

Overview *Starting rate $19.50/hour* The Day Services Case Manager will provide an environment that fosters growth and independence through therapeutic and recreational services and will assist in the design of a developmental skills training program with the interdisciplinary team. Develop, implement and monitor collaborative therapeutic service plans, completing all necessary documentation and reporting significant concerns to the supervisor as needed. Assess individuals’ behaviors and complete daily data, monthly progress notes and quarterly reviews. Foster community integration and provide support to individuals to actualize this goal. Qualifications Bachelor’s Degree; or High School Diploma and two years’ experience working with individuals with developmental disabilities. Strong interpersonal skills and ability to use good judgement. Excellent written and verbal communication skills. Basic computer knowledge. Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients. Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
