Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

Must be a graduate of an approved school of clinical education.
Must be licensed in the state where they currently practice.
Must have at least one year experience as a licensed professional.
Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
Medicare home health or hospice experience is preferred.

Requirements

Must possess a valid state driver license
Must maintain automobile liability insurance as required by law
Must maintain dependable transportation in good working condition
Must be able to safely drive an automobile in all types of weather conditions
Must possess CPR certification for the healthcare provider

Monitor patients closely during infusion treatments, promptly addressing and managing any complications or adverse reactions. Manage complications associated with infusion therapies, such as adverse reactions or infusion site issues. Demonstrate expertise in the insertion, care, and maintenance of intravenous (IV) lines and central venous devices. Ensure compliance with infection control practices and standards in the management of vascular access devices. Complete ONS/ONCC Chemotherapy Immunotherapy Certification within the first year of employment. Maintain AHA BLS certification. Performs the lead job duties Patient Assessment and Management of Care: Conduct comprehensive assessments of patients before, during, and after infusion therapy to evaluate their overall health and response to treatment. Provide thorough education to patients and their caregivers on the purpose of infusion therapy, potential side effects, and self-care measures. Work with other members of the multidisciplinary team to ensure patients receive comprehensive education and proper resources tailored to their care needs. Lab Testing and Result Management: Analyze and interpret laboratory reports related to patients' oncology status, ensuring timely communication of findings to the healthcare team. Monitor pertinent lab results to assess patients' eligibility and qualification for specific therapies, collaborating with the provider to adjust treatment plans accordingly. Maintain certifications on all lab equipment to perform Point-of-Care (POC) testing for quick turnaround of CBCs, INRs, urinalysis testing, and blood glucose testing. Demonstrate expertise in conducting POC testing, ensuring accuracy and reliability of results for immediate clinical decision-making. Uphold quality control measures and adhere to regulatory standards in performing POC testing. Documentation and Record Keeping: Maintain accurate and up-to-date patient records, documenting infusion therapy details, vital signs, nursing assessments, and interventions. Ensure adherence to documentation standards and regulatory requirements. Patient Care: Identifies opportunities to improve patient care based on feedback from staff and customers.
