Responsibilities:

  • Utilize the nursing process to assess, plan, implement, and evaluate patient care.
  • On each assignment, provides skilled nursing care/services in accordance with prescribed orders
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
  • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.


Qualifications:

  • Currently licensed as an LVN in the state in which the LVN will practice.
  • Current TB or Chest X-Ray.
  • Current BLS card.
  • One year prior professional nursing experience preferred

Description Cornerstone Specialty Hospitals Conroe is a long-term acute care (LTAC) hospital specializing in the treatment of patients recovering from post-intensive care and medically complex conditions. Our hospital provides both intensive care and telemetry-level services, offering advanced monitoring and support for patients requiring extended medical care. With a multidisciplinary team dedicated to personalized treatment plans, we focus on helping patients achieve significant recovery and transition to the next level of care. Job Summary Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plans in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharge planning process. Essential Functions Maintains the standard of nursing care and implements policies and procedures of the hospital and nursing department.
