We are seeking a compassionate and skilled Evening Shift LPN to join our interdisciplinary team at Valley Manor Rehabilitation and Healthcare Center in Coopersburg, Pennsylvania. As a valued member of our staff, you will play a vital role in providing high-quality, patient-centered care to our residents.

About the Role:

As an Evening Shift LPN, you will be responsible for delivering exceptional care to our patients while promoting their physical, emotional, and spiritual well-being. Your duties will include, but are not limited to:

Providing direct patient care, including administering medications, wound care, and vital sign checks
Developing and implementing individualized care plans in collaboration with the healthcare team
Educating patients and their families on disease management, treatment options, and self-care techniques
Participating in interdisciplinary team meetings to discuss patient care and progress
Maintaining accurate and detailed records of patient care and treatment
