About the Role

SYNERGY HomeCare Northeast Metro is looking for compassionate, dependable Care Professionals to provide quality in-home care to clients throughout the Northeast Metro.

You'll provide personalized support based on each client's individualized Plan of Care, helping them remain safe, comfortable, and as independent as possible in their own homes.

What You'll Do

Your responsibilities may include:

  • Providing compassionate, respectful care based on each client's individualized Plan of Care

  • Assisting with activities of daily living, including bathing, dressing, grooming, toileting, feeding, oral hygiene, and bedtime care

  • Providing mobility assistance, including ambulation, exercise, and range-of-motion activities

  • Providing companionship and positive social interaction

  • Assisting with permitted hands-on care such as support stockings, ace bandages, and hot/cold therapies

  • Providing medication reminders only—Care Professionals do not administer medications

  • Observing clients and reporting changes in condition, safety concerns, incidents, or accidents

  • Accurately documenting care provided and services rendered

  • Maintaining a clean, safe, and comfortable client environment

  • Working professionally with clients, families, supervisors, and fellow Care Professionals

  • Maintaining client dignity, confidentiality, and appropriate professional boundaries
