Job Qualification

  • Graduate of an accredited school of practical nursing or of a Nursing Vocational or Nursing Technical school required.
  • Current License to practice as a Practical Nurse in New York State required, plus specialized certifications as needed.

JOB SUMMARY: The Clinical Assessor / Coordinator, LPN is responsible for reviewing sub-acute care and long-term care referrals received from surrounding areas hospitals/facilities. The Clinical Assessor / Coordinator, LPN identifies actual and potential mental and physical health problems and accurately determines the necessary classification and regimes to assist with the appropriate placement of a potential admission to the Gurwin Jewish Health Care System.
