Responsibilities:

Assist clients with daily living activities, such as bathing, dressing, and grooming, in a safe and respectful manner
Provide personal care, including toileting, transferring, and ambulation assistance as needed
Monitor and report changes in clients' condition to licensed nursing staff
Assist with medication reminder.
Develop and maintain positive relationships with clients and their families, fostering a supportive and inclusive environment
Collaborate with interdisciplinary teams to develop and implement individualized care plans, promoting effective communication and coordination of care
