Job Description:

  • Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals. 
  • Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space.
  • Communicates through various digital channels to support patient care.
  • Performs initial health screens and intake for new members.
  • Obtains and reviews old medical records to ensure comprehensive patient history.
  • Identifies care gaps and orders necessary screening tests.
  • Confirms appointments and provides instructions to patients.
  • Conducts pre-visit RAF reviews to prepare for patient visits.
  • Welcomes patients, provides tours of the practice, and reviews welcome packets.
  • Ensures all medical records are available for office visits.
  • Performs needed screenings and orders appropriate tests during visits.
  • Conducts intra-visit RAF reviews to assess patient needs.
  • Reviews quality metrics, performs vitals, screenings, and medication reconciliation.
  • Manages the check-out process with patients and conducts post-visit follow-up calls. 
  • Other duties as assigned.
