Role Overview

As a Home Health RN, you are the clinical lead for your patients - managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge.

 

You’ll combine advanced assessment skills, care planning, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home.

 

What You’ll Do

  • Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
  • Develop and manage individualized care plans in collaboration with physicians and care teams
  • Deliver skilled nursing care, including medication management, treatments, and interventions
  • Serve as the case manager, coordinating care across therapists, aides, and physicians
  • Monitor patient progress and make clinical decisions based on changing conditions
  • Educate patients and families to support disease management and independence
  • Coordinate care transitions and discharge planning to reduce readmissions
  • Maintain timely, accurate documentation in the EMR
  • Supervise and support Home Health Aides as needed
  • Participate in on-call rotation as appropriate

 

What You Bring

  • Active RN license (or compact eligibility) + CPR certification
  • Graduate of an accredited nursing program
  • Minimum one (1) year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred)
  • Strong clinical judgment and ability to think independently
  • Excellent communication skills and patient-centered approach
  • Ability to manage a caseload and prioritize effectively
  • Proficiency with EMR systems
  • Valid driver’s license and reliable transportation

Primary Responsibilities: Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition Documents problems, appropriate goals, interventions, and patient/family response to hospice care Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Current unrestricted RN licensure in the state of practice 1+ years of clinical experience Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation State Specific Requirement: LA: a minimum two years full time experience working as an RN. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse Preferred Qualification: Current CPR Certification or ability to complete within 90 days of hire Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
