Nurse Practitioner (APRN)– Home Health Support

The Lakes Home Care is seeking a licensed Nurse Practitioner (APRN) to support our home health operations. The NP will play a key role in ensuring compliance with Medicare guidelines, assisting with Plan of Care (POC) documentation, and conducting Face-to-Face patient encounters.

Responsibilities:

• Review and sign Plan of Care (POC) and recertifications

• Perform Face-to-Face encounters as required

• Support clinical documentation compliance (Medicare/AHCA)

• Collaborate with RNs, MDs, and administrative staff

• Assist in reducing hospital readmissions through patient follow-up

Requirements:

• Active Florida APRN license

• Active NPI number

• Medicare enrollment (PECOS) preferred

• Experience in Home Health or geriatrics preferred

• Knowledge of Medicare guidelines and documentation

Compensation:

• Per visit: (depending on service)

• POC review/signature: negotiable

• Monthly contract option available

DEPARTMENT ADDENDUM Department Specific Functions The Executive Health Department provides convenient and personalized attention to patients in our care. The APRN is responsible for assessing physical and psychosocial status of patients in an outpatient clinic setting by means of interview, health history, physical examination, and diagnostic studies. Assist physicians in performing medical procedures, up to and including blood drawing, vital signs and nursing assessments, history, and physical. This position requires travel to multiple clinic locations. Collaborates with physicians regarding patients to assess, plan, implement and evaluate patient care. Provides direct care, counseling, and teaching to patients. Performs physical examinations and provides preventive health measures appropriate to patient needs. Develops, implements, and communicates plans of care in a timely manner to patient and family, as well as the appropriate team members, ensuring compliance with all regulatory guidelines. Facilitates staff, patient and family decision making by providing educational tools. Triages patient calls as needed to determine if patient needs to be seen or can provide guidance over the phone.
