Responsibilities:

• Provide direct patient care to residents, including vital signs, personal care, and mobility assistance
• Assist with ADLs (Activities of Daily Living) such as bathing, dressing, and grooming
• Maintain accurate and up-to-date records of resident care and treatment plans
• Collaborate with the healthcare team to develop and implement individualized care plans
• Report changes in resident condition or concerns to the RN/LPN on duty
• Maintain a clean and organized living environment for residents
