Role Summary:

As a DAY 7a-7p LPN, you will play a vital role in delivering high-quality care to our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans, administer medications, and provide wound care. Your exceptional communication skills will enable you to effectively educate residents and families on their care, while your attention to detail will ensure accurate documentation and record-keeping.

Responsibilities:

Administer medications and treatments as prescribed by physicians
Provide wound care and manage wound healing
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Conduct routine vital signs and monitor resident conditions
Document resident care and progress in electronic medical records
Communicate effectively with residents, families, and healthcare team members
Maintain accurate and up-to-date records of resident care and medications
Perform other duties as assigned by the RN or DON

Requirements:

Current Florida LPN license
High school diploma or equivalent required
Previous experience in a long-term care setting preferred
Ability to lift and transfer residents as needed
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Reliable transportation and flexibility to work varied shifts
