Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

  • Must be a graduate of an approved school of clinical education.
  • Must be licensed in the state where they currently practice.
  • Must have at least one year experience as a licensed professional.
  • Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

  • Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
  • Medicare home health or hospice experience is preferred.

Requirements

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • Must possess CPR certification for the healthcare provider

Capable of using in-home virtual technology to connect with the Patient Care Hub and conduct virtual visits for patients in the home. Transports and delivers items, equipment, and medications (excluding controlled substances and IV chemotherapy drugs) as requested. Qualifications Graduate of an approved school of nursing. Meets all mandatory, developmental, and performance competency requirements for Baptist Health and unit/department. 1 year experience required unless approved by AVP level or above. Hospital Care at Home – Two (2) years acute care nursing experience required. Maintains current State of Alabama RN license or valid eNLC multistate RN license. Maintains current BLS/Healthcare Provider certification. ACLS, NRP, PALS, TNCC are required for certain areas. NRP required for Neonatal Intensive care Unit (NICU). Must possess a valid driver’s license and has reliable transportation to travel to perform job duties for Hospital Care at Home In- home nursing.
