Job Summary:

As a Licensed Practical Nurse (LPN), you will play a vital role in our care team, working closely with patients, families, and other healthcare professionals to provide high-quality care. We are seeking a dedicated and compassionate LPN to join our team on a part-time and full-time basis.

Responsibilities:

  • Administer medications and treatments as ordered by physicians

  • Monitor and record patients' vital signs and medical conditions

  • Develop and implement individualized care plans with input from the healthcare team

  • Perform routine nursing tasks, such as wound care and vital sign checks

  • Communicate effectively with patients, families, and healthcare team members

  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • Current LPN licensure in the State of New Jersey

  • Associate's degree in Nursing or related field

  • Minimum 1 year of experience as an LPN in LTC

  • Ability to lift and transfer patients with assistance as needed

  • Strong communication and interpersonal skills

Job Description OFFICE VISITS: Introduces self to patient while escorting patient into the exam room. Prepares the patient for exams and procedures. Explain treatment procedures, medications, diets and physicians' instructions to patients. Administers medications including all types of injections, IV therapy as needed and within the scope of licensure. Provide basic patient care and treatments, such as but not limited to: dressing wounds, performing catheterizations or applying compresses. Collects, labels and processes specimens accurately. Cleans and prepare medical treatment rooms for patient visits. Assemble and use equipment appropriate to specific medical practice. Schedule follow-up appointments for patients as needed. Communicates delays effectively when necessary. CHARTING: Enters patient information into the computer accurately and in a timely manner. Manages the physician's desktop accurately and in a timely and efficient manner.
