We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at The Gardens at Country in Swansea, Massachusetts. As an LPN at our facility, you will play a vital role in providing top-notch care to our patients and residents, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the healthcare team
• Communicate effectively with patients, family members, and healthcare professionals to ensure seamless care transitions
• Participate in quality improvement initiatives to drive excellence in patient care
• Maintain accurate and detailed records of patient care and treatment
