Job Summary

Responsibilities

Provide personal care services to clients, including bathing, dressing, grooming, and toileting

Monitor and report changes in client condition to the nursing team
Maintain accurate and detailed records of client care and services provided
Work collaboratively with the nursing team to develop and implement individualized care plans
Ensure a safe and clean environment for clients

Requirements

Current certification as a Certified Nursing Assistant (CNA) or Home Health Aide
High school diploma or equivalent required
Ability to work in a fast-paced environment and prioritize tasks effectively
Excellent communication and interpersonal skills
Ability to lift, transfer, and reposition clients as needed
Work well under the direction of supervisors and healthcare team members
